We discussed teach back will improve patient understanding, decrease errors, decrease call backs, improve patients’ outcome. First, we will evaluate each patient medication reconciliation through phone or face to face interview. Then, we will educate patients clearly and use teach back method to state back in own words. Last, we will evaluate the patients’ medication reconciliation accuracy. We may use quantitative method to present the results before and after clear education for medication reconciliation accuracy. We will encourage to use Orlando’s deliberative nursing process theory. Her deliberative nursing process theory focuses on the interaction between the nurse and patient, perception validation, and the use of the nursing process to produce positive outcomes or patient improvement (Faust, 2002).
