Please provide a synthesis of the following: the at-risk population, the resources utilized, and how the chosen referral program assisted in accessing the resources by the at-risk population noted.

Part 3

Using one of the two programs chosen, assess how this program will provide

referral to an intervention to serve the at-risk population. Please provide a synthesis of the following: the at-risk population, the interventions utilized, and how the chosen referral program assisted in accessing necessary interventions.

Please note Part 2 and 3 do not have to serve the same at-risk population.
