*** Use the topic of your project as the ‘subject line’ for your post (not your name or the name of the assignment).

(1) Survey Information: Include at the beginning of the survey:

• brief description of the topic and definitions of terms

• instructions for answering the survey

• active consent

(2) Survey: Write your completed survey with all questions and answers, ready to be answered. (Do NOT include an answer key or any scoring instructions or any potential diagnosis or interpretation of the results for the takers of the survey).

(3) Justification: Write 10 separate paragraphs, one for each of the 10 questions on your survey, to justify the wording used for each question, justify the inclusion of each survey question, and explain how each question fits in with the other questions and within your topic. Explain why each question should be part of the survey and how it gives insight into the topic you have chosen.

(4) References: The sources you used in APA format related to the topic of your project. Use at least three PRIMARY sources to support your work. Do not include the textbook.

NOTE: All references must be about your topic only – not about Likert Scales.
