Describe caring attributes of the culture where you currently practice. Which attributes stand out as having significant influence on patients, nurses, and other healthcare professionals?

In my current practice caring attributes can be seen through clinicians who are motivated and moved to practice compassion and respect for all patients. Just recently, we had a patient who has been on our services for quite some time who was admitted into the hospital. The concern was for the patient when he returned home from the hospital. His home was very cluttered, and not the most sanitary. The patient did not have any assistance with housekeeping. When you walked in his house, your feet would stick to the carpet from all that had been spilled on it. The patient was wheelchair bound and extremely obese. The nurse and the nursing aide, with permission from the patient, went into his home while he was in the hospital and cleaned the clutter and carpets to make his home safer upon his return home. The nurse and the home health aide were moved with compassion and empathy for this patient. According to the ANA standards of professional performance, the registered nurse will practice with “compassion and respect for the inherent dignity, worth, and unique attributes of all people”. (ANA, 2015). In a study of nursing students in a baccalaureate nursing program, Rhodes et al (2011) identified a foundational value for nursing is caring. (Rhodes et al., 2011). Caring was the essential characteristic expressed by over two thirds of the students and many believed it was the most important trait nurses should possess. (Rhodes et al, 2011). According to Adams (2016) caring demonstrates a reflection of high regard toward patients as human beings. Nurses care for patients across the continuum from illness to wellness and must be approached holistically. (Adams, 2016). According to Gelinas (2017) a culture of caring can be identified as a professional practice that works to improve the health of patients and/or community. (Gelinas, 2017). Nurses can make significant contributions to a patient’s experiences, safety and healing and impact patient outcomes through the very nature of caring. (Gelinas, 2017).

How do you see effective communication relating to patient outcomes in this setting? What is the evidence for this?

Effective communication has an impact on the rapport clinicians build with patients and family members. Being able to effectively communicate establishes a basis for trust. According to Chou (2018) effective communication will lead to better satisfaction of patients, increases trust between patients and clinicians, and increases the functional and psychological well-being of patients. (Chou, 2018). According to Chou (2018) evidence shows that effective communication “leads to improved outcomes in specific diseases, including a small but significant absolute risk reduction of mortality from coronary artery disease, improved control of diabetes and hyperlipidemia, better adherence to antihypertensives, bereavement adjustment in caregivers of cancer patients, and higher self-efficacy of adherence to HIV medications”. (Chou, 2018). Chou (2018) also states that when communication is ineffective, the compliance of patients can be negatively impacted, and hospital readmission rates are increased. Through effective communication, nurses teach patients about their disease process and medications they are taking. Effective communication is not only about speaking but entails the ability to listen and reflect as well.
